In susceptible individuals, smoking causes inflammation of the tunica intima of small vessels by an unknown mechanism, which results in thrombotic occlusion of the vessel. Patients initially present with a classic triad of intermittent claudication, Raynaud phenomenon, and migratory thrombophlebitis. Eventually, critical limb ischemia develops and the patient presents with rest pain, absent pulse in the extremities, and/or digital gangrene. Ultrasonography and arteriography are used to localize the site of occlusion and differentiate TAO from other causes of peripheral artery disease. The most important therapeutic measure is the complete cessation of smoking. Additionally, prostaglandin analogs (e.g., iloprost) may be used to improve blood flow and decrease rest pain. Patients with TAO who develop digital gangrene require amputation.A skin biopsy can be helpful although the features are not specific for thromboangiitis obliterans. The histology usually shows intraluminal thrombosis (a blood clot within the vessel) with associated microabscess formation. A mixed infiltrate of white blood cells and giant cells may be seen in all layers of the vessel wall, although the internal elastic lamina is classically spared and helps distinguish thromboangiitis obliterans from other types of vasculitis.

Thromboangiitis obliterans (Buerger disease) is caused by small blood vessels that become inflamed and swollen. The blood vessels then narrow or get blocked by blood clots (thrombosis) Thromboangiitis obliterans produces segmental inflammation in small and medium-sized arteries and, frequently, in superficial veins of the extremities. In acute thromboangiitis obliterans, occlusive thrombi accompany neutrophilic and lymphocytic infiltration of the intima; endothelial cells proliferate, but the internal elastic lamina remains intact. In an intermediate phase, thrombi organize and recanalize incompletely; the media is preserved but may be infiltrated with fibroblasts. In older lesions, periarterial fibrosis may occur, sometimes affecting the adjacent vein and nerve.
